Those skills basically fall under the jurisdiction of a "Spellsword". Some would call it a Battlemage but this seems incorrect, a Battlemage mainly relies on spells for combat and can use a melee weapon up close for support but a Spellsword does it the opposite way, using spells to support a melee oriented combat style. Both can use Conjuration for support and distractions but Battlemages typically use ranged damage spells in the form of Destruction magic, since you didn't list Destruction as a skill this has to be a Spellsword.
