I don't in game or lore references, but I can use real life to help your questions.
1) It has been said in the Fallout universe that Nuka-Cola was very addicting and once you had one you would crave another. Just like in real life once you drink one or two coca-colas you brain becomes rewired to crave it and need forming an addiction.
2) Chems were addicting though not as quite as Nuka-Cola, but only if you excessively used them or you were prone to addictions. Just like in real life there have been studies where two people each smoked a pack of cigarettes and only one of them became addicted.
3) Using real world again Caffeine and Sugar, two main ingredients in both Nuka and Coca, are 10x more addictive than Morphine or in Fallout's case Med-X.
