How does the gear work. Per the wiki it appears they allow "layering" but I am wearing a set of clothes and got a leather chest piece that overrides the clothes when I try to equip it. Not sure I'm clear on how gear works in general to maximize defense and protection for self.

There are certain items that are thin enough to be worn under armor. The Vault suit, Road/Raider leathers, and Fatigue variants (Dirty Army Fatigues are my favorte). Take a look at the Special boosting stats for these as they can be helpful depending on your character.
As for maxing defense, there is Ballistic and Energy Resistance as the main two. If you look at leather vs. metal armor, leather has better energy resistance, Metal has beeter Ballistic damage. Combat armor (which you'll find later) has equal in both.
So depending on your build, you can start planning out armor type.

Do you use that pretty much 100% of the time after you get it?
Also, please tell me there is an easier way to break down items to their craftable materials...I read that you have to drop them and then scrap them individually. Holy hell that took 1/2 an hour for the first batch.

oh boy - YES to the second! Just store everything in your workbench....just plop it all in there. When you go to construct, the workshop breaks it down for you!
As for Power Armor - it's your playthrough preference. You don't need it. It's super powerful, but there are downsides...espcecially in Roleplay

So having the same "Bethesda" issues with FO that I did when I started Skyrim. Knowing where to put my perk points while leveling. Since this is my first playthrough I'm "winging" it but a TON of stuff I don't know.
1. Weapon damage always seems to be lagging. Even adding some points to mods I'm still in the 20-30 range for damage for most (low 50's max). Ammo isn't a huge problem but the issue is it's mostly .38 which blows. 2-3 shots of 10mm is six or seven shots of .38. Hard to do with ghouls swarming you (those boogers are fast too!).
2. Armor: Better to go with the shield buff or the lightning bolt buff? (Not really sure what they do exactly.)
3. Followers...better with or without? The dog seemed helpful but my first human...seems to not shoot so much.
4. And as noted, just knowing which perks flow together to give the best results.
